Forms are VERY important! Especially when it comes to getting your permit. Follow the instructions below, have all your documents in order, make an appointment with the DMV to take your permit exam and you should have no problems.

Forms You'll Need To Present at the DMV

 1. Submit identification establishing age, identity and lawful presence in the United States such as:

(Photocopies or notarized copies are not accepted)

  • Certified State or County Birth Certificate.
  • United States Certificate of Birth Abroad.
  • Previous License or Identification Card expired less than 1 year.
  • Valid Passport.
  • Valid Military Identification Card.
  • Resident Alien Identification Card.
  • Naturalization Certificate.
  • Valid Foreign Passport with Immigration Visa/I-94. (No Border Crossing Cards or a Visa status of B1, B2, W/T, W/B, C/P or N/C)

2. Provide your Social Security Number (SSN).

  • If you state that you do not have an SSN, you must provide documentation from the Social Security Administration supporting your statement.

3. Supply a Colorado residence address.
